Tina Lonneman

I have been a nurse practitioner for 24 years and have practiced in Women’s Health, Family Practice, Endocrinology and Bariatric Medicine/ Obesity Medicine.​

While my time spent in my previous medical practices was rewarding and provided me with a wealth of knowledge and experience, I decided to leave the traditional healthcare system so I could incorporate a more lifestyle medicine approach. 

I envisioned a practice where I was able to combine my years of clinical experience as a healthcare provider with my passion for women's health, nutrition, fitness, and health education.

I founded Elevate Health with the aspiration to help others achieve optimal health through educating, coaching and supporting them along their journey!

Unfortunately, with lack of good information along a plethora of misinformation it is no wonder that people are feeling confused and overwhelmed when making health related decisions.

Obesity is complex and often multifactorial thus the reasons that it may difficult for some individuals to lose weight.

We aim to provide up to date information (science is always changing) to assist with weight loss and support maintenance.  

Our primary approach is educating and empowering our patients to make significant lifestyle and behavior changes.  We will also prescribe FDA approved weight loss medications if indicated.  

NO “fad” diets… as evidence supports that they do not often lead to healthy and sustained weight loss.

Educating and Empowering Midlife women to optimize their health during the menopause transition

If you are a woman experiencing perimenopause and menopausal symptoms we can help you navigate your path through the hormone changes. While the average age of menopause in the US is 52, some women may start having symptoms of perimenopause in their mid 30's.  The physiological and psychological changes and symptoms that women experience during perimenopause can vary significantly from woman to woman. Some women may have only mild annoying symptoms while others have significant disruption in their quality of life. Having undergone additional training in menopause medicine we fully understand the that hormonal changes during the menopause transition have the potential to impact almost every organ system in your body including  brain, genitourinary, cardiovascular, metabolic  and musculoskeletal systems. ​

We can help you build a midlife wellness plan including nutrition, physical activity and other lifestyle changes to optimize your transition to menopause.

We will also provide extensive education on hormone therapy and other medications to treat menopause related symptoms.  We discuss the options, risk/benefits of HT,  assess your individual risk factors, and help you decide if hormone therapy is right for you.  We make every attempt to choose HT options that are covered by your insurance.

Unfortunately, the risk for insulin resistance, pre-diabetes, diabetes, high cholesterol,  and other metabolic health conditions increases as we age.  The good news is that there are strategies that can reduce the risks.  Optimizing your nutrition and other lifestyle habits can have a significant impact on your health span! 

Have you already been diagnosed with a metabolic health disorder or you think your nutrition is sub-optimal? If you want professional guidance to improve your nutrition and prevent disease and optimize your health we can provide nutrition coaching.   We strive to help you sort through all the dietary and nutritional information that is so confusing.  Because most of the “nutrition guidelines” are outdated you will be provided with up to date, evidence based nutrition advice.  

 

We understand that each person is unique and has certain dietary preferences, potential food allergies/intolerances, and medical conditions that necessitate an individualized nutrition plan. There is no “right” eating plan…regardless if you prefer to incorporate more plant based or animal based, we will assist you in building a nutrient dense eating plan. 

Because a behavioral approach is also important to making successful changes we also focus on mindful eating and helping change habits that may have inhibited you from being successful in the past. 

Fitness Assessment & Exercise Prescriptions

Physical activity prescriptions for healthy aging
Prevent sarcopenia -Exercise is medicine

Sarcopenia (loss of muscle mass/function) is typically thought to be only in the aging population.   However, it is becoming more common in younger individuals due to poor dietary habits and decreased physical activity. As mentioned, sarcopenia can also occur in individuals who lose a large amount of weight (because both fat and muscle is often loss during weight loss).  Sarcopenia can lead to significant health consequences including increased risk for fragility, disability, fractures, and loss of independence and overall decreased quality of life.

For people of all ages …. it is never too late to improve your physical fitness.
 

If you currently feel intimidated to either go to a gym or work with a personal trainer, OR have just been reluctant to start lifting weights/resistance training because you do not know how to start we can provide a safe, non -judgmental space for you to start with the basics.   At Elevate health we can coach and assist you with the aim of helping you build confidence in yourself and motivate you to start your fitness journey!! 

While people are aware of the importance of cardio -respiratory fitness many do not know the positive impact of resistance training. Muscle is now referred to as the “organ of longevity” and for good reason! Having more muscle can benefit your health in so many ways beyond appearance….including improve metabolic, mental and immune health just to name a few!

We want to inspire and assist you to build more muscle and provide education on the importance of treating and preventing sarcopenia as you age. 

We can also assist individuals who have had bariatric surgery, planning to have surgery, OR those  desiring non surgical weight loss.  Our goal is to help you maintain and even build your muscle mass during your weight loss journey.  We also educate on the importance of a progressive exercise routine to help prevent regain and improve overall health.   

 

Based on initial history we will determine if more advanced medical clearance is indicated prior making basic recommendations.   If any significant concerns are found or further expertise is indicated a referral to physical therapists, orthopedics, or other medical professionals will be made.

Offering both in person and virtual visits (through a secure HIPPA Telehealth platform).

There are limited appointments available online booking. We offer flexible scheduling.  If you do not see a time that works for you let us know and we will work with you to find a time that is convenient for you! We even offer evening and Saturday appointments.

We currently accept all major credit cards ( including FSA/HSA) and cash & checks. 

We currently do not accept insurance and payment is due at the time of service. 
We will provide you with a superbill if requested an you can submit to your health insurance for possible reimbursement.

We encourage you to use your health insurance for medications and blood work and make every effort to prescribe medications covered by your insurance.

Initial consultations: 


$175.00   Typically 60-75 minutes. 

Done in person if possible but will do Telehealth if you live outside the south Denver area or just prefer to not meet in person. 

During your first visit we will perform a thorough review of your health history. Discuss your symptoms and health concerns.  If you have had recent blood work done with another provider we will review to obtain a better understanding of your current health status. If indicated we will order any necessary lab or diagnostic testing.  

Together we build a wellness plan including nutrition, physical activity, lifestyle changes, and prescriptions to help you feel better and achieve your health goals. After your consultation we have a patient portal to communicate as often as needed in between appointments.

Follow up visits:  $110.00  typically 30- 40 minutes

Package deals also available and can be discussed at your visit.
